My Health Record

My Health Record is an online summary of your key health information that has been developed by the Australian Government.  Like other information stored on myGov accounts you can log in to see basic key health information all in the one place.  It is flexible because you can access this information wherever you have access to the internet.

To see what is in a person’s My Health Record you need to create an account or sign in to myGov  

The first time you try to access My Health Record through myGov, you will need to link it with myGov and create a record for use by health professionals.

As a carer, you can create a My health record for the person you care for.  If you need any help with setting up the record or using it, please ring: 1800 723 471

What information is stored in My Health Record?

Health professionals can add clinical documents to a record. Medicare and PBS information held by the Department of Human Services can also be added.  You can also add documents so that they can be shared amongst health professionals when needed.  Examples of documents are listed below.

Documents that may be included in My Health Record are listed below.

Health Professionals may choose to upload the following:

  • a GP can upload a health overview, called a shared health summary, which can be useful for visiting new health professionals
  • hospital discharge summaries
  • reports from tests and scans, like blood tests
  • prescribed medications
  • referral letters from your doctor(s)

(Please note: It is optional whether a health professional uploads information.  Some doctors and health professionals may choose not to upload information.)

Medicare/ PBS can upload the data from the previous 2 years including:
•information held by the Department of Human Services;
•information stored by the Department of Veterans’ Affairs;
•organ donation decisions and;
•immunisation records from the Australian Immunisation Register, including childhood and other immunisations received. 

You can upload information too

You can upload information too so that it is all in the one place and can be accessed by other health professionals.

  • contact numbers and emergency contact details
  • current medications
  • allergy information and any previous allergic reactions
  • Indigenous status
  • Veterans’ or Australian Defence Force status
  • an advance care plan or contact details of the person who is the holder of the advance care directive/living will (the custodian).
